DEADLINE is Thursday, October 31: Biometric Screenings for COVA Care and COVA HealthAware Participants and Spouses
By completing a biometric screening, an employee or spouse enrolled in these plans will be able to receive a premium reward of $17 each for the months of January 2014 thru June 2014
Biometric screenings must be completed between August 1 and October 31, or have been done at a physician’s office between April 1 and October 31, to count toward premium rewards for the second half of the plan year. Remember that you also must complete the health assessment to qualify for premium rewards. To access the health assessment participants will need to register at https://www.myactivehealth.com to create an account and once you are log in you may access the health assessment tab.
